It contains information regarding contact people, service or carer provider information, medications and any medical conditions. This book may be accessed by family members, carers, friends or emergency services if required.

This book was developed via a combined Community Service Project by Rotary and Lions Clubs and Ambulance Services. All people, adults and children who take medications or have a medical condition or disability should have a book.
Each book has a clear plastic sleeve with strip magnets and is placed on the fridge door once it has been filled out by the owner (patient) together with their doctor.
